It would seem that this device is going to be a new member to the Droid Incredible family: Droid Incredible 4G.
This 4g enabled phone surfaced in February and some leaked documents show it may land sometime this month.

There is a new member to the EVO family at Sprint. Yesterday came the HTC EVO 4G LTE.
This phone will be one of Sprint's first available 4G LTE devices feature an 8MP camera on the back, and a 1.3 shooter on the front. Also features a 1.5GHz Snapdragon processor running Android 4 and Sense 4.
The phone will be available for pre-order May 7Th 2012.
